Hak Pasien Patient Rights
a. Mendapatkan Informasi
To Receive Information
Berhak mendapatkan informasi mengenai kondisi kesehatannya sendiri dan penjelasan yang memadai tentang pelayanan kesehatan yang diterimanya.
The patient has the right to receive information regarding their own health condition and an adequate explanation of the health care they receive.
b. Pelayanan Berkualitas
Quality Services
Berhak menerima pelayanan kesehatan yang sesuai dengan kebutuhan medis, standar profesi, dan standar pelayanan yang berkualitas.
The patient has the right to receive health care that meets medical needs, professional standards, and quality standards.
c. Menolak Tindakan Medis
Refusing Medical Treatment
Berhak menolak atau menyetujui tindakan medis, kecuali untuk tindakan yang diperlukan dalam rangka pencegahan penyakit menular atau penanggulangan wabah.

d. Akses ke Rekam Medis

Kewajiban Pasien Patient Obligations

Wajib memberikan informasi yang jujur, lengkap, dan akurat mengenai masalah kesehatannya sesuai kemampuan dan pengetahuannya.
The patient must provide honest, complete, and accurate information regarding their health problems to the best of their ability and knowledge.
b. Mematuhi Nasihat Tenaga Kesehatan
Complying with the Advice of Health Care Professionals
Wajib mematuhi instruksi, nasihat, dan petunjuk yang diberikan oleh tenaga medis dan tenaga kesehatan.
The patient must comply with the instructions, advice, and directions given by medical and health care professionals.
c. Menaati Peraturan Fasilitas Kesehatan
Complying with Healthcare Facility Regulations
Wajib menaati ketentuan, peraturan, dan tata tertib yang berlaku di fasilitas pelayanan kesehatan.
Must comply with the provisions, regulations, and rules applicable to healthcare facilities.
d. Memberikan Imbalan Jasa
Providing Compensation
Wajib memberikan ketidakseimbangan atau jasa atas pelayanan kesehatan yang telah diterimanya.
Must provide compensation or compensation for healthcare services received.
e. Mematuhi Rencana Terapi
Complying with the Treatment Plan
Wajib mematuhi rencana terapi yang direkomendasikan oleh tenaga kesehatan dan disetujui oleh pasien setelah mendapat penjelasan.
Must comply with the treatment plan recommended by the healthcare professional and agreed to by the patient after receiving an explanation.
2. Selama dalam perawatan di Dokter Praktek Mandiri MutiaraCare pasien bersedia dilakukan pemeriksaan dan tindakan medis, keperawatan serta pemeriksaan penunjang lainnya.
During care in Dokter Praktek Mandiri MutiaraCare patient agree to examination and medical treatment, nursing care and diagnostic examination.
3. Selama perawatan di Dokter Praktek Mandiri MutiaraCare pasien yang memerlukan tindakan medis dan pengobatan akan diberikan penjelasan oleh tim medis yang merawat sebelum pasien menyatakan persetujuannya untuk dilakukan tindakan atau pengobatan tersebut.
During care in Dokter Praktek Mandiri MutiaraCare patient need medical treatment and medication will give explanation by medical team before patient declare agree to do it.
4. Penjelasan terkait pengobatan maupun tindakan medis yang akan di lakukan sesuai poin 3 termasuk menyetujui risiko medis yang bisa terjadi apabila timbul komplikasi dari tindakan atau pengobatan yang tidak diharapkan.
Explanation regarding the treatment or medical action that will be carried out according to point 3, including agreeing to the medical risks that may occur if unexpected complications arise from the action or treatment.
5. Selama dalam perawatan di Dokter Praktek Mandiri MutiaraCare pasien dianjurkan untuk tidak mengenakan atau menyimpan barang berharga. Kehilangan ataupun kerusakan barang bukan merupakan tanggung jawab manajemen RSUP Sanglah Denpasar.
During care in Dokter Praktek Mandiri MutiaraCare patient advice not to bring or use valuebles. Hospital is not liability for any damage or lose of valuebles.
6. Pasien dan keluarga bersedia mengikuti peraturan dan ketentuan yang berlaku.

7. Menyetujui untuk saling menghargai sebagai antar-manusia secara profesional dan bermartabat sederajat.
Agree to respect each other as human beings professionally and with equal dignity.
8. Apabila pasien atau siapapun yang datang ke wilayah Dokter Praktek Mandiri MutiaraCare melakukan kegaduhan ataupun tenaga medis/tenaga kesehatan/staf Dokter Praktek Mandiri MutiaraCare terasa terganggu ataupun terancam, setuju untuk dikeluarkan, dibebaskan hak-haknya, dan dilaporkan ke pihak berwajib bila diperlukan.
If a patient or anyone who comes to the Dokter Praktek Mandiri MutiaraCare area causes disturbance or the medical personnel/health workers/staff of Dokter Praktek Mandiri MutiaraCare feel disturbed or threatened, agree to be removed, their rights waived, and reported to the authorities if necessary.
9. Selama dalam perawatan di Dokter Praktek Mandiri MutiaraCare pasien dijamin privasi dan kerahasiaan penyakitnya

10. Pasien memiliki hak yang sama dalam hal untuk memperoleh akses informasi kesehatan, melepaskan informasi kepada petugas dan informasi tentang tata cara mengajukan keluhan.

Apabila Dokter Praktek Mandiri MutiaraCare membutuhkan proses hukum untuk menagih biaya pelayanan Rumah Sakit dari saya, saya memahami bahwa saya bertanggung jawab untuk membayar semua biaya yang disebabkan dan proses hokum tersebut.
If the Dokter Praktek Mandiri MutiaraCare requires legal proceedings to collect fees for Hospital services from me, I understand that I am responsible for paying all costs incurred and the legal process.
